If you are getting ready for your first visit we want you to know a few things:
1. We are excited to meet you and trust your time in physical therapy will be helpful and healthful!
2. Please bring the following items to your initial evaluation:
- insurance cards
- a doctor’s prescription (if required, call us if there is a question)
- a current list of medications you take, including dosages
- your copay
3. Please remember that we are in a pool environment. Your evaluation will not be in the water but it is warm in here. Please wear comfortable, loose clothing.
